Please read this Privacy Notice ("Notice") to understand how BENEV Company, Inc. ("BENEV," "we," "us," or "our") collects, uses, and shares your personal information, and the choices and rights you have. This Notice applies to everyone who interacts with us, including both healthcare professionals and consumers or patients who visit our websites to learn about our products, purchase products, submit a Provider Locator request, or otherwise contact us.
By using our websites or submitting information to us, you acknowledge the practices described here. If you do not provide information we identify as necessary, we may be unable to provide the service you have requested — for example, completing a purchase or matching you with a local provider. We may update this Notice from time to time (see Section 11).
01 Sources of personal information
We collect personal information from and about you through the following channels:
Directly from you.
Information you provide when you complete a form (such as our Provider Locator), contact us, subscribe to communications, attend an event, or otherwise interact with us.
Automatically.
Information collected as you use our websites, through cookies and similar technologies (see Section 4).
From third parties.
Information from our affiliated companies, service providers, healthcare providers in our network, event partners, analytics and advertising providers, and publicly available sources.
02 Information we collect
Depending on how you interact with us, we may collect the following categories of personal information:
Contact and identifying information.
Such as your name, email address, phone number, and ZIP or postal code.
Inquiry and interest information.
The products or treatments you tell us you are interested in, your preferred method and time of contact, and any details you include in your message. Because your treatment interests may relate to your health, we treat this as sensitive information.
Purchase and transaction information.
If you buy products from us, information related to your order — such as billing and shipping address, order history, and the products purchased. Payment card details are collected and processed by our third-party payment processor; we do not store full payment card numbers.
Professional information (for healthcare professionals and business contacts).
Such as your practice name, role, credentials, and history of interactions with BENEV.
Device and usage information.
Such as IP address, browser and device type, operating system, and how you navigate and interact with our websites.
We will indicate where information is required rather than optional. We do not require you to provide more than is necessary for the purpose you have chosen.
03 Children's Privacy
Our websites and services are intended for adults and are not directed to children. We do not knowingly collect personal information from anyone under the age of 16. If we learn that we have collected such information without appropriate consent, we will delete it promptly. If you believe a minor has provided us information, please contact us using Section 12.
04 Cookies and similar technologies
We use cookies and similar technologies to operate our websites, remember your preferences, measure site performance, and — where permitted — support analytics and advertising. You can manage your preferences through our Cookie Settings and your browser controls. For details on the technologies we use and their purposes, see our Cookie Policy. Where we use hosted web fonts, we configure them to serve without transmitting your IP address to the font provider wherever technically possible.
05 How we use your information
We use your personal information for the following purposes:
To process and fulfill your purchases.
To take and fulfill orders, process payments, arrange shipping and returns, provide order support, and maintain records of your transactions.
To respond to you and match you with providers.
To process your Provider Locator request, identify providers near you, respond to inquiries, and follow up with you about your request.
To communicate with you.
To send you information you have requested and, with your consent where required, marketing about our products and services. You can opt out of marketing at any time.
To operate and improve our websites and services.
Including analytics, troubleshooting, and product development.
To maintain relationships with healthcare professionals.
Including verifying professional information and managing business communications.
To meet legal, safety, and compliance obligations.
Including product safety and adverse-event reporting, recordkeeping, responding to lawful requests, and protecting our rights, property, and the safety of others.
06 Provider Locator and how we share your information
When you submit a Provider Locator request, you understand and agree that we will share the information you provide — such as your name, contact details, ZIP code, and stated treatment interest — with a limited number (typically one to three) of independent providers near you, so that they may contact you directly.
Once your information is shared with a provider, that provider becomes an independent controller of your information and its own privacy practices apply. BENEV does not control how a provider uses the information you choose to share with them thereafter.
More broadly, we may share your personal information with:

Independent providers in our network, as described above, to fulfill your request.

Our affiliated companies, including our parent company, ExoCoBio Inc. (Republic of Korea), for the purposes described in this Notice.

Payment processors and fulfillment partners, who process your payment and ship the products you order. Payment card details are handled by our payment processor under its own security standards.

Service providers, who process information on our behalf and under our instructions — for example, website hosting, form processing, communications, and analytics providers — and who are required to keep it confidential and secure.

Legal and transactional recipients, where required by law, in connection with legal proceedings or lawful requests, or in connection with a merger, acquisition, financing, or sale of assets.
We do not sell your personal information for money. Certain data-sharing for advertising or analytics may be considered a "sale" or "sharing" under California law; see Section 9 for how to opt out.
07 How long we keep your information
We keep your personal information only as long as necessary for the purposes described in this Notice, including to respond to your request, maintain our relationship with you, comply with our legal and recordkeeping obligations, and resolve disputes or enforce our agreements. When information is no longer needed, we delete it or de-identify it. Retention periods vary by data type and purpose; specific schedules are maintained in our internal records policy.
08 Data security and international transfer
We use reasonable administrative, technical, and physical safeguards designed to protect your personal information against unauthorized access, use, or disclosure. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.
BENEV is based in the United States. Because our parent company and certain service providers are located outside the United States — including in the Republic of Korea — your information may be transferred to, stored in, or accessed from countries whose data-protection laws differ from those in your jurisdiction. Where required, we put appropriate safeguards in place for these transfers.
09 Your privacy rights
Depending on where you live, you may have some or all of the following rights regarding your personal information. California residents have specific rights under the California Consumer Privacy Act, as amended (CCPA/CPRA):

To exercise these rights, submit a request through our Data Request Webform, email us at Info@BENEV.com, or call (949) 919-6799. To opt out of the sale or sharing of your information, use our "Do Not Sell or Share My Personal Information" link. We will verify your identity before acting on a request and will not discriminate against you for exercising your rights. You may use an authorized agent to submit a request on your behalf, subject to verification. If we deny your request, you may appeal by contacting us at the address in Section 12.
10 Third-party links
Our websites and communications may contain links to third-party sites and services, including providers' own websites and social media platforms. This Notice does not apply to those third parties, and we are not responsible for their privacy practices. We encourage you to review their privacy notices.
11 Changes to this Notice
We may update this Notice from time to time to reflect changes in our practices or legal requirements. When we do, we will revise the "Last updated" date above and, where appropriate, provide additional notice. We encourage you to review this Notice periodically.
